Assignment 2: Capstone Practicum Assignment: Phase 4: Implementation Plan

Purpose:

The implementation process moves the strategic plan from a dust covered book into practical steps that propel changes. You only have one module to implement your care plan, so begin small—say, one or two families, or a small group of 5–10 people.

Tasks:

By the due date assigned, submit a 3- to 4-page implementation plan addressing the following items:

• Identify a Change Theory that you would use to implement your proposed solution.
• How do you plan to implement this intervention? Do you have any specific strategy in mind? What resources will you need?
• Describe the process for implementation using SMART goals.
• Identify at least 4 outcome variables you would use to measure or evaluate success.
• How long do you think it will take for the results to be seen?
• Explain how the proposal solution or implementation utilized leadership, communication, and collaboration skills to improve a clinical problem in your setting.
• Support your responses with examples. Cite all sources in APA format.
